Social Work Services Department
The Social Work Services Department at Eden Medical Center is made up of a team of licensed clinical social workers through the Board of Behavioral Sciences of California. Their primary goal is to meet the varying needs of patients and families in crisis. They are your source for answers to general questions and have a great knowledge of valuable community resources.

Advance Healthcare Directive/POLST
The experts from our social work team can assist you with the formulation of Advanced Health Care Directives (PDF) when needed and offer assistance with navigating through the sometimes daunting and complicated healthcare system.
The experts from our social work team are also an invaluable resource with assisting families with the formation of Physician Orders for Life Sustaining Treatment, otherwise known as POLST.

Spiritual Care Services
Eden Medical Center is pleased to provide the services of a hospital chaplain. If you or your family members wish to speak to a chaplain, please ask your nurse to contact the Spiritual Care Coordinator to arrange for a visit, or you may leave a message at ext. 1619.
The chaplain is available to provide spiritual guidance, read from scripture, talk through a difficult decision, provide sacrament, anointing or blessing. They are also happy to pray with you and/or family members.
Eden Medical Center also provides a meditation room located on the first floor adjacent to the ICU. To locate the room, follow signs at the lobby elevators or ask a hospital staff member for assistance.
