    Send a Thank You Note

    Would you like to recognize an Eden team member who went above and beyond to provide you with excellent care? You can send a quick note of appreciation to an individual, a department, or the entire medical center.

    Let us know the special care or service you received as a patient or visitor. If you have the individual's name, please include it, as well as any other details about your care (dates you were in the hospital, etc.). All the information you share will be kept confidential in compliance with our privacy policy. Based on the information provided, we will route your message to the appropriate person or department.

    Patient Satisfaction Survey

    We appreciate your feedback on the care and service you received while at Eden Medical Cneter. Your comments, suggestions, compliments and concerns help us improve the quality of our services. Within a few weeks after discharge, a random selection of patients receive our Patient Satisfaction Survey. If you receive a survey, please take the time to complete it and return it to our survey company.
