    Your safety and care at Eden Medical Center are of greatest importance to our staff. We encourage you to actively participate in your care by asking questions and sharing any concerns you might have.

    Sharing Your Concerns

    If you have concerns related to care, treatment, services and/or patient safety issues, we encourage you to follow the steps below to ensure we are providing the best care possible.

    • Talk with the manager of the area in which you are being treated. He or she will work with you to resolve the issue to your satisfaction. You can also contact Administration at (510) 727-2703.
    • If you feel your concern has not been resolved to your satisfaction, you can file a Statement of Concern by calling (510) 727-2712 or by requesting the form from the unit manager. This will ensure you receive a formal written response to your concern. In any event, you will be contacted by Eden Medical Center as soon as possible, but at least within three business days.
    • You may contact the California Department of Public Health at 850 Marina Bay Parkway, Building P, 1st Floor, Richmond, CA 94804, (510) 620-3900, or The Joint Commission's Office of Quality Monitoring at 1-(800) 994-6610 for concerns about patient safety.
    • For concerns about accommodations for patients or visitors with disabilities, please contact Eden Medical Center's ADA director at (510) 727-8270.
    Please be assured your action will in no way adversely affect the care you are receiving or your access to care.

    Patient Satisfaction Survey

    After your stay at Eden Medical Center you may receive a written survey with questions about a variety of services and interactions. Your response to the survey allows us to share compliments and improve our services.
